We Bill Your Insurance Directly for Eye Exams
Direct billing makes covering the cost of your family’s eye exams, glasses, and contacts simpler.
We offer direct billing for all major insurance providers. Instead of you paying out of pocket and then waiting to be reimbursed, we bill your benefits company directly. If there’s a balance remaining after your benefits have been used, that’s the only cost you pay.

Where is Clarity Optometry located and where do I park?
We are at 21 College Ave West, Guelph, ON N1G 1R7, next to the Guelph Campus Co-operative and across from the OVC Small Animal Clinic. We offer plenty of parking right in front of our clinic. To access our lot, turn onto Borden St., drive behind the building, and circle around to park in front.
What are your hours?
We are open Monday from 8:00 AM to 8:00 PM, Tuesday from 8:00 AM to 7:30 PM, and Wednesday to Friday from 8:00 AM to 5:30 PM. We are closed on Saturday and Sunday.
Do you bill my insurance directly?
Yes. We offer direct billing for all major insurance providers for eye exams, glasses and contact lenses. Who are the optometrists at Clarity Optometry?
Our team includes Dr. Paul Fasulo, Dr. P. Lynne Leis, Dr. Reita Thomas-Parel, Dr. Violet Zawada-Kuzio and Dr. Thiru Shathasivam, supported by certified optometric assistants. You can read each optometrist’s background on this page.
What languages do you speak?
Our team speaks English, French, Polish and Italian, and Dr. Shathasivam also speaks Tamil.
Do you see children, and is their eye exam covered?
Yes, we provide eye care for patients of all ages, including children’s eye exams. Your child’s annual eye exam is covered by OHIP from birth through age 19. We also participate in Ontario’s Eye See…Eye Learn program, which provides 1 pair of glasses per child when they are enrolled in junior or senior kindergarten and an eye exam shows they need glasses.
What eye care services do you offer?
We offer comprehensive eye exams for adults and children, diabetic eye exams, contact lens exams and fittings, dry eye treatment, myopia control for kids, emergency eye care, eye disease diagnosis and management, digital eye strain support, laser eye surgery consultations and eye nutrition.
How do I book an appointment?
You can book by calling us at 519-822-2710 or by filling out the request form on our Location & Appointments page.
Can I buy glasses and contact lenses at your clinic?
Yes. We carry frames and lenses and sunglasses, and our experienced optometric assistants will help you find something tailored to your needs. You can also reorder contacts through our online store.
